New GP Chronic Condition Management Plan (GPCCMP)

From 1 July 2025, the Enhanced Primary Care (EPC) and Team Care Arrangement (TCA)

programs have been replaced by the GP Chronic Condition Management Plan

(GPCCMP). This update streamlines access to Medicare-subsidised allied health

services for people living with chronic conditions.


What’s Staying the Same

  • Patients remain eligible for up to five allied health visits per calendar year.
  • These visits can be used across one or multiple allied health disciplines.


What’s Changed

  • Simplified Referrals: GPs now issue a standard referral letter—just like a specialist referral. No more EPC/CDM forms.
  • No Team Collaboration Required: GPs can refer directly without needing to coordinate with other providers.
  • Longer Referral Validity: Referrals are valid for 18 months from the first service, unless otherwise specified.
  • Focus on Ongoing Care: The new plan encourages regular GP check-ins to support long-term condition management.


Who Can You See?

Eligible allied health professionals include:

  • Aboriginal health workers/practitioners
  • Audiologists, Chiropractors, Diabetes educators
  • Dietitians, Exercise physiologists, Mental health workers
  • Occupational therapists, Osteopaths, Physiotherapists
  • Podiatrists, Psychologists, Speech pathologists


What You Need to Do

  1. Talk to your GP: Discuss your chronic condition and eligibility for a GPCCMP.
  2. Get a referral: Your GP will provide a standard referral letter—either to you or directly to your chosen provider.
  3. Choose your provider: Take your referral to any eligible allied health professional.
  4. Check your visit count: You are responsible for checking your remaining allied health visits via My Health Record.


Important Note for Enhance Movement Clients

If you have already used all five Medicare-subsidised allied health visits with other providers, you will be responsible for the full session fee at the standard rate when attending an Exercise Physiology consult at Enhance Movement. Please check your visit status on My Health Record before booking.
